| Community Outreach Advisor |
This is a free and confidential service for older people living in Rushcliffe to help them remain safe, happy and healthy in their own homes. The Community Outreach Advisor is able to visit you in your own home and to talk with you about what help and support you feel you need. Whether it is information or advice on getting help with transport, your pension, adaptations to your home or what local groups and clubs exist, the Community Outreach Advisor will try to put you in touch with the right help and support.
"Just also wanted to say a great big THANK YOU for all you have done for the patients referred so far. They have really appreciated your visits, and the input and referrals made. It has been really successful and made such a difference to them and their families. From my point of view, you have been a star and I know that the patients are getting a far better service and signposting into services than I could initiate myself"
Liz Harris, Community Matron
How does it work?
Case Study:How the Community Outreach Advisor Service helped Mrs PMrs P, aged 99, lives alone in the family home in a rural village. She has family nearby who help her out but does not leave the house from day to day and is very isolated and lonely. She is registered blind and walks very slowly round the house with a frame, using the dining room as a bedroom. The Community Outreach Advisor visited on three occasions to discuss her needs. Mrs P was referred to a physiotherapist to give her some seated / bed-based exercises to help her mobility. She was also referred to a local lunchclub and appropriate transport was arranged for her. This proved to be a real boost to her confidence, as she met several previous acquaintances there. She was also put in contact with the Notts Royal Society for the Blind who were arranging a fortnightly visit to provide her with some company, help her with correspondence etc and to help her with the “Articles for the Blind” equipment she had been loaned.
